No one will be able to answer this question. It is clear that we learn both words and grammar, so store in the head. Through various processes, these are retrieved and combined when we actively use language, so write, read or listen. How exactly the individual processes work is the subject of psycholinguistic examination. Here are different models that can all explain more or less much. Research in this field has not been completed for a long time. If you are really interested in the topic, I can recommend this book:
