How do spam comments actually get into YouTube videos?

Let me try to explain it better: Under many videos, there are one or two comments where the profile picture is a woman's abdomen in underwear. This comment always has something to do with sex.

But the videos seem to be random. From eco-tests to gaming to beauty channels.

Why do they appear so randomly? Is someone responsible? Or is this some kind of global virus? ^^ (also increasingly seen on TikTok and Instagram)
