Addicted to benzos and no help from a psychiatrist, what to do?
I take alprazolam for my panic attacks. I've tried many antidepressants, but none have helped me. Only with alprazolam can I lead a halfway decent daily life. I'm desperate; should I keep taking alprazolam forever? The psychiatrist told me that the only medication that could help me was Lyrica; instead, he prescribed me another SSRI antidepressant. What should I do?
Alprazolam is a drug from the group of benzodiazepines which is used primarily for the brief treatment of severe psychiatric anxiety. As emergency medicaments, benzodiazepines are alternativeless, but for continuous treatment they are not suitable or even counterproductive.
On the one hand, there is a high risk of dependence on excessive consumption, on the other hand there is a development of tolerance. Will heissen: The more common and the higher you dose the tablets, the faster they lose their effect. In the worst case, one is completely dependent on it without having a positive effect. You only need the tablets to avoid developing withdrawal symptoms.
Other drugs are therefore used for the longer term drug treatment of anxiety. The first choice is SSRI and SNRI antidepressants. These generally act relatively reliably (not always) but do not depend. They have other disadvantages: antidepressants must be taken daily, only after 3-5 weeks and can have numerous side effects especially at the beginning of the treatment.
As an alternative to the SSRI and SNRI antidepressants, the older tricyclic antidepressant Opipramol is also used. It has a different mechanism of action than the newer SSRI’s and SNRI’s. Thus, even in the case of ineffectiveness of the latter, there is the possibility of the opipramol acting. There are numerous other tricyclic antidepressants which act in anxious manner (e.g. doxepine or clomipramin). However, these are rarely used due to their strong side effects.
A newer alternative is Pregabalin (Lyrica). Pregabalin is an anxious antiepileptic which must also be taken daily or several times a day. An effect results after about 1-3 weeks. It again has a completely different mechanism of action. Pregabalin can also have numerous side effects, especially at the beginning of the treatment. As a rule, however, it does not depend. However, there are individual case reports of people who have been dependent on pregabalin. People with addiction diseases (especially opioids) are primarily concerned in their history.
Well, what can you do if your psychiatrist doesn’t want to prescribe you? In addition, it is up to the doctor to decide which medicine he prescribed. Since you seem to have not addressed antidepressants, there would theoretically be a medical reason to try with pregabalin. However, as already mentioned, the decision lies with the doctor. If your doctor refuses to prescribe the necessary medicine, you have to accept it or change the treating doctor.
