It can be perfectly normal to wake up immediately when you get the reality check at first, because you're mostly used to it not working. When it suddenly works, it can be so surprising that you wake up.

It's best to always do reality checks with the expectation that they'll work. This can help prevent you from being too surprised in your dream.

Yes, that's correct. After observing your surroundings, ask yourself whether you might actually be in a dream, and when you do the RC, assume you can breathe.

By the way, it can sometimes happen that reality checks in dreams don't quite have the desired effect. The RC you're doing is the one I use most often myself. I've noticed that the result in dreams can be negative if you have a stuffy nose while sleeping. Nevertheless, I still find this RC the most accurate compared to others.
