Very severe pain is not what one hopes for physiotherapy. This can be a disintegration, a bloodshed or an irritated nerve. If you have a calcified place in the muscle, it can also occur.
Actually, treatment should not lead to such pain. If heat helps, keep it warm. You can also call the therapist again, but another treatment will probably only be done if the current problems have disappeared in a few days.
