The extrarenal elimination fraction (Q0) of a drug indicates which proportion of the total elimination of the drug does not take place via the kidneys. If the kidney function is impaired, the dose adjustment may be necessary to avoid excessive enrichment of the drug.

The dosage for a patient with reduced kidney function can be calculated using the Cockcroft-Gault formula or other specific dosage recommendations for the respective drug. The Cockcroft-Gault formula uses serum-creatinin values, the age and sex of the patient to estimate the glomerular filtration rate (GFR).

However, without specific information about the drug, the dosage units or the route of administration, it is difficult to propose an accurate dosage adjustment. It is important that the dosage is determined individually for each drug and every patient taking into account all relevant factors.
