They are caused by long-lasting sexual excitation without subsequent sperm. In this case, the sperm strands and secondary hodes swell and press on the testicles.

The cavalier’s pain goes back after a few hours when the swelling has declined.

Easy, but harmless.

Medical treatment is generally not necessary. However, a sperm may reduce the duration of the pain somewhat.

I can’t judge that. Itching is generally rather superficial, so on the skin.

That should have nothing to do with the testicles.

Sometimes testicle pain can also radiate from the abdomen to the testicles. However, this can only be judged by a doctor, for example an internist.
