Is it possible that caries is not visible from the outside?

I'm currently seeing a new dentist, and he said I need a tooth filled. The damage is only visible on an x-ray; nothing is visible from the outside. The tooth in question doesn't hurt, looks great, and is a premolar, so where there could be a surface for caries to form is a mystery to me.
Is it really possible that caries cannot be seen from the outside?
