Itchy testicles when in contact with something?

I (M14) have had the following problem for about 10 months: my testicle constantly itches when it comes into contact with my underwear, and if I scratch it only gets worse. I have been to my family doctor twice who prescribed me antifungal ointment and once to the dermatologist who prescribed me some other ointment, but nothing has helped.

Then a week ago I switched to cotton underwear in the hope that it would get better, but so far nothing has changed.
