I'm watching a series on Netflix or TikTok and my iPhone volume is about halfway up, and suddenly, without the volume changing, the sound gets much quieter. Then I turn the volume up to full volume, and it's as if I had the volume at medium. But after two minutes or so, the sound gradually gets louder again.

Whether the tone has been set to the middle does not matter. It depends on the volume.
