How do I get the best substitute?
First, hello, about me. I'm addicted to opiates/opioids. I'm 17 years old and I'm from Munich, where things are already very difficult when it comes to drugs. Especially with minors. (Don't worry, my parents have already taken away all my money/blocked my account and will only let me out if they know who I'm with and check this by calling the person.) But when you're addicted, you'll always find a way to get drugs if you're stupid.
My first detoxification therapy will begin soon and will last at least nine months. Before that I will have to go into withdrawal for about two weeks at the Heckscher Clinic, which I don't have a good impression of but haven't heard anything good about either. I have asked them many times if they can give me some kind of substitute or anything that will help me stay clean because I was clean after cold turkey at the time. However, they said they can only give me pipamperon, which I absolutely hate and can't tolerate at all. I told them that, but they said I should keep taking it. I've been back on it for a while now (morphine and sometimes oxycodone) because it's taking ages to get a place in rehab. I have an appointment with my psychiatrist soon (I was seeing her before she found out about the drugs, about two years) and I just wanted to ask her if she can prescribe me Subutex or something similar. Even pregabalin, which gives me a rash, as I was prescribed it once in Vienna (I would even put up with the rash, and it isn't pretty). However, she is in contact with the clinic and they think that pipamperon does work, despite repeated denials. I'm simply concerned that I would rather get my "stuff" in a regular manner and without having to go out on the streets. Not even a dose that gets me high, but simply so that I don't go crazy and want to kill myself, and I just can't take it and go crazy without it. My parents are okay with it too. At the moment they even give me 200mg of morphine in the morning and 300mg of pregabalin in the evening so they can monitor it and I don't overdose, as they unfortunately had to find me that way recently, but luckily I survived. I don't get high from the stuff I voluntarily gave to my parents either. So it's been working pretty well so far, only it will all be gone in about 3 days. The problem is that I can't tell my parents about it at the appointment with my psychiatrist because they would also be committing a crime, even though they are only trying to help me so that I don't suffer another overdose.

Your parents are subject to confidentiality. Can you talk about it?
That’s why I ask you for your advice, what I can do or you can say, because it’s quite a hurry, and I have the appointment in exactly 3 days and so it’s as good as everything depends on her. I have also asked my family doctor about Substi /Prega, but he has said very clearly no.
I’m asking for answers, advice or ideas there’s no longer much time and my parents, friends and I don’t know anymore.

The path in a reliable substitution – provided that parents agree – leads via personal contact to a drug consultancy, which then conveys you to a substitution doctor if necessary.
However, it’s total bullshit to “create again” when your reconciliation therapy starts anyway. If the upstream withdrawal takes place in clinical care “cold”, you will probably regret to put such a crash on the day.
Good improvement. Sometimes it can take a few years to get out of opiate dependence, unless you accidentally administered the letal dose, which would not be unusual in times of fentanyl as opiate substance.
