How do I create these small note tables in Word (see image)?

It is difficult to estimate due to the image detail. In principle, several possibilities are conceivable.

Possibility 1: Hang and format two rows and X columns below

Possibility 2: … and I think more about this because the middle line 1 is not continuous and 2 is not exactly at a height when you look closely.
