Is the factor 4.88 justified when preparing a cavity?

Hello, my dentist recently repaired a few carious cavities. Now my dental insurance won't cover the factor stated on the bill. A factor of 4.88 is listed for almost all cavities. I don't have any bad teeth, and the caries was actually very moderate. Is my dentist's factor justified?

An excerpt from the invoice:

Cavity preparation and restoration with composite materials, using the adhesive technique, two-surface, including multi-layer technique if necessary, including polishing, and if necessary, including the use of inserts. Reason: Considerable time expenditure due to the particularly difficult filling design in the contact area with the adjacent tooth. Above-average difficulty and time expenditure due to the poorly located, difficult-to-reach cavity. FACTOR 4.84

Over 3.5 is only possible with a fee agreement. So you would have to have specifically signed that you agree with it and pay the difference yourself. You got that?

This is where only freedom of contract is concerned. If both of them are to be paid, then that’s okay.
