Can I write a polite sick note like this?

I was just about to send my doctor's note to my teacher. I want to make the email as friendly as possible, especially because I'm feeling guilty about missing an exam :,) (although I'm really sick!). Especially since it's the second time with this teacher…

Would it be excessive to write something like “Thank you for your understanding” in addition to the usual “I enclose the certificate…” etc.?

I don’t think a teacher has an understanding for a subsequent medical report, there is not even a liberation, I read from the question.

To courtesy You are not obligated to report a disease. If there is no uniform formal path within the school, how to send medical messages, then you can make it formless. Example:

Dear Mrs. Zipfelmus,

I had to go for medical treatment because of health complaints and will be missing a few days at school. I’ll send them my medical certificate.

[Your name]
