Help filling out L16?

So,

I received an L16 form from the tax office and have to fill it out.

I've read it a thousand times, but I still can't get to grips with it.

Can someone help me with this.

What and how should I fill out the lower section.

Seems to be Austria and I think the service owner / employer must fill this out.
